#3 - CAUSE

This is the reason.

  • It is an explanation of why the non-compliance occurred. It addresses the root cause of the discrepancy. It discusses what was incomplete, inaccurate, or an activity that was not performed. The cause is not always known. 
  • It answers the question: What led to the noncompliance?

QUESTION:  Think back to the last example of milk and orange juice being served as a snack. What is a possible root cause? 

 

Here are a few possible reasons: 

Reason # 1

LACK OF TRAINING 
The cook was new and maybe did not know the meal pattern.

Reason # 2

LACK OF OVERSIGHT
The cook made the substitution and no other staff member reviewed the revised menu to ensure the substitution made met the CACFP meal pattern requirement.

Reason # 3

LACK OF RESOURCES
The cook was following the menu given to her, milk and orange slices but made the substitution to fit what she had available.
