6 Steps of the Serious Deficiency Process

Follow the 6 sD Steps

This is an overview of the 6 steps of the serious deficiency process.  The sponsoring organization must follow these steps if it is determined that the non-compliance rises to the level of serious deficiency.

The 6 steps of the serious deficiency process are:

STEP 1

Identify the serious deficiency.

STEP 2

Prepare and issue the Notice of Serious Deficiency (NSD) and request written corrective action.

STEP 3

Evaluate the day care home’s corrective action plan.

STEP 4

(A) Accept the correction action and issue the Notice of Temporary Deferment (NTD), or (B) deny the corrective action and issue the Notice of Proposed Termination and Disqualification (NPTD) and the organization’s Appeal Procedures.

STEP 5

Hold an administrative review (appeal).

STEP 6

(A) If termination and disqualification are upheld, issue the Notice of Final Termination and Disqualification (NFT), or (B) if termination and disqualification are overturned, issue the NTD.
